As a part of the Provider Directory team, the Provider Installation Representative 2 will be responsible for building forms as one of their main tasks; typically completed in June and July each year for the following year. The provider information has to be maintained and remain accurate, so our members have a positive experience in obtaining the needed information for their wellbeing; therefore, this tea reviews the directories (in PDF form) every 30 days.

Hours: Monday – Friday Shift, First 90 days 8:00am – 4:30pm
Flex time after probation, “office” hours 6:00am – 5:30pm, Must log in by 9:00am; 40 hours – with potential overtime June – October
Training: Remote with camera, Intermittent for 8 weeks “classroom” instruction with trainers, working tasks
Location: Remote KY or Remote IN, live within 90 miles of Louisville, KY

Work at Home Connectivity Requirements
